Home Health Service – Provide home health aides, skilled nursing care, companion services or even help with household chores

Long Term Care – For seniors with chronic and/or progressive conditions, such as Parkinson’s disease, a debilitating stroke, or Alzheimer’s disease

Short Term Care – For someone is recovering from an illness, injury or surgery in order to help patient make smooth transitions from the hospital back to their homes.

Adult Day Care – Offer structured activities and programs which provide a chance for the patient to be social and to participate in staffed activities such as music and exercise programs.

Long Term Care Categories

Low Dependency

Independent or semi-dependent requiring minimal assistance in activities of daily living

• Mobile
• Wheelchair bound
• Minor sores (grade 1-2) & bruises
• Self-feeding or spoon feeding
• Able to self-clean or with minimal assistance
• Early-Stage Dementia

Medium Dependency*

Requires full support for activities of daily living but not critically ill

• Bed bound
• Moderate to advanced dementia
• Mild to moderate suctioning frequency
• Ryle’s tube feeding
• Grade 3 bed sore
• Mild-Stage Dementia
• Grade 3 bed sore

High Dependency*

Requires skilled nursing care and more attention from the staff, usually fully dependent

• Grade 4 bed sore or multiple grade 3 bed sores
• Oxygen dependent
• Tracheostomy
• Advanced palliative care
• Risk of infections
• Suprapubic catheter, pig-tail catheter, other forms of tubing besides feeding & urinary tube
• Late-Stage Dementia
